Client Application Process and Forms

The first step in the application process it to download the PDF Restore House application packet at the bottom of this page, fill it out, and send it in. (You can do so by way of emailing the document or by faxing it.)

The following documents are for the application process. Please read and fill the out before returning the documents to Restore House:

•         General Consent Forms
•         Program Outline
•         Application Packet

The final document is an informational flyer. If you choose to print this out, please keep it for your own use.
